Dependent Verification Review
Your American Fidelity account manager can help you make sure that your medical insurance costs are as accurate and efficient as possible by conducting a review to verify the eligibility of participating dependents according to your negotiated contract language. This is a beneficial service to many employers, helping to reduce costs by eliminating non-qualified dependents; reduce plan usage, often leading to fewer future premium increases; and reduce overall risk experience.
During the review, we will:
- Review submitted employee documentation, such as: marriage licenses, birth certificates, college transcripts, first page of current tax forms, state certificates, guardianship papers.
- Conduct private, individual interview sessions with all benefit-eligible employees.
- Obtain a signed “Dependent Eligibility Form” from every benefit-eligible employee. Forms must be signed by both the Account Manager and employee to verify information accuracy. The signed forms are then given to the employer along with a report of the completed review.
- Provide periodic progress reports and status updates, a well as Subsequent Maintenance Reviews to continue to validate dependent eligibility. Maintenance reviews can be conducted annually based upon pre-determined criteria to ensure regular reviews of each employee’s dependent eligibility.
